Dr. Keith Warr and his staff are continuing an exceptional family dentistry practice that has spanned 50 years. The tradition of providing patients with exceptional dentistry started in 1957 by his father, Dr. Newell Warr, and has continued with Dr. Keith Warr for over 15 years. Dr. Warr’s primary concern is to provide you with a personalized, pleasant cosmetic dentistry experience so you will never hesitate to address any of your dental or oral health needs.

Dr. Warr and the staff believe in practicing preventative dentistry. We have all worked hard to provide our patients with the best products and services currently available. Warr Dental Office Manager
Adina Miano

Adina is committed to providing patients with outstanding customer service and quality of care. It is her hope and expectation that during your time spent at Warr Dental, your needs are met promptly and professionally, always with care and sensitivity. Her greatest joy is helping patients have a fantastic experience through her attention to detail, patience and positivity.

Adina joined the Warr Dental team in 2008, bringing over 15 years of experience in the dental field. Originally from the San Francisco bay area, Adina has been living in Salt Lake City since 1999.

Outside of work, she enjoys spending time with her husband Bill and their two dogs, Chloe and Roscoe. She also takes pleasure in photography, traveling and literature.

Hygienist
Chelsea Hacking

Chelsea Hacking is originally from Orem, Utah and is excited to be back in Utah after five years of living in Colorado. Chelsea completed for pre-dental hygiene course work at UVU, her clinical dental hygiene work at Sheridan College in Sheridan, Wyoming and her Bachelor's degree through the University of Wyoming. Chelsea practiced dental hygiene in Boulder, Colorado for five years before moving back to Salt Lake City in May of 2006. She has thoroughly enjoyed working at Warr Dental and being part of such a progressive, patient centered practice. When Chelsea is not working, she loves to travel, especially to destinations by water. Chelsea loves independent film, continued study of the French language, and being bossed around by her Standard Poodle, Cosette.

Hygienist
Jenn Berrett

Jenn is blessed to be the mother of three beautiful children and wife of a loving husband. Her hobbies include vacationing anytime, anywhere. She finds satisfaction in using her creative talents by scrapbooking and decorating. She keeps busy by attending her little boy's sports and occasionally getting pampered with her little girl. When she is not playing the role of Mother/Wife, she enjoys working as a hygienist at Warr Dental since 1994. She graduated from SLCC in 1998 and feels fortunate to be working at Warr Dental for over 10 years. She enjoys the quality of dentistry and preventative care that is offered here. She also enjoys building longtime friendships with the patients she serves and looks forward to many more years to come.

Dental Assistant
Janice Turner

Janice was born in Fort Worth, Texas. She has been married for 30 years and has two wonderful sons and daughter-in-law. She was recently just blessed with her first granddaughter Brooklyn Louise.

Her hobbies include spending time with her family and any kind of arts and crafts.

Before moving to Utah, Janice had the opportunity to work for the Las Vegas Institute for Advanced Dental Studies (LVI) where she received extensive training and knowledge in cosmetic and neuromuscular dentistry. She has worked in the dental field for 34 years and is passionate about caring for patients and their well being. She is a valued member of the Warr Dental team.

Dental Assistant
Katie Lewis

Katie Lewis is originally from Fruit Heights, Utah. She attended Davis High School, followed by Dixie College and Weber State College. She originally started with Warr Dental in 2005, worked for two years, moved away for school and now has recently returned. She loves her job and enjoys the friendships of her co-workers along with the many patients she cares for. Outside of work, she and her husband love anything to do with the outdoors. She has a passion for snow skiing and snowboarding in the winter.

Come Spring/Summer, she enjoys water sports along with road biking. She has a 10 month old Golden Retriever named Jack that keeps her busy while not at work.

Dental Assistant
Nicole Rashleger

Nicole Rashleger has been in the dental field since 2007, when she joined the team at Warr Dental. Her efforts as a dental assistant are unsurpassed. Her dedication to her patients is remarkable.

This California native has been living in Utah since 1994. When she is not helping her patients she enjoys traveling, shopping, and spending time with friends and her loved ones.

Scheduling Coordinator
Amy Davis

Amy has been with Warr Dental since March 2009. She is originally from Holladay, Utah, where she graduated from Olympus High School followed by Salt Lake Community College. Amy previously worked as a Marketing Director for a construction and developing company. Amy enjoys interacting with our patients and is a valued member of the Warr Dental team. "It is so rewarding coming to work each day knowing that what we do here will positively change the lives of our patients forever".

Outside of work, Amy loves her kickboxing and Pilates workouts as well as a good long hike outdoors.
